Your first visit
The single commonest reason people put this off for months is not the cost or the waiting time. It is embarrassment, and a page that skips over that is not being much use to anybody.
The awkward part
Almost everybody says some version of the same sentence in the first few minutes, which is that they have been meaning to come in for a long time and feel ridiculous about having left it. It is so consistent that it is worth treating as a design problem rather than as a personality trait.
So the visit is built around it. One clinician in the room and nobody else. The examination lasts a couple of minutes and you are covered throughout it. Everything is said out loud as it happens rather than afterwards, because not knowing what comes next is most of the discomfort.
And nothing you describe will be the worst thing heard that week. The examination page describes the physical part in detail precisely so there are no surprises, the symptoms hub covers how common all of this actually is, and itching covers the complaint people find hardest to say out loud.
Practicalities
Most of it is conversation. The examination is a couple of minutes of that, and you are dressed again immediately.
Everything, including anything bought over a counter. Blood thinners in particular change what can be done.
Sent in advance if possible, so the coverage answer is ready rather than being worked out while you wait.
Creams, wipes, supplements, how long each went on. This changes the plan more often than people expect.
What you leave with
Four things, and they are worth listing because a visit that produces fewer than four has not really done its job. The grade, or the reason a grade does not apply. Which options that allows. Roughly how many sessions a course would run to. And what it will cost you.
Nothing at all is booked on the spot unless you want it booked. A good number of people take the figures away and think about it for a week, and a few decide the conservative route is enough for now, which is a legitimate outcome rather than a failed appointment.
If any of the four is missing when you leave, that is worth going back for. What treatment costs covers the fourth, the treatments hub covers the second, and referrals covers what happens if this needs someone else.

Including the ones people would rather ask anonymously than at the desk.
The first visit was questions, an examination, and a plan, all in forty minutes. I left knowing the grade and what came next.
Nothing was treated until the examination was done. The doctor explained each step before doing it.
The intake form was short and nobody asked me to repeat the embarrassing parts out loud at the desk.
I was told at the first visit how many sessions were likely and what each would cost. Both held.

Get started
Say what the symptoms are in a sentence and roughly how long they have gone on. That is all that is needed to get the appointment usefully set up.
If the visit shows this needs a different specialty, you will be told that there and then.
